Thank you for wanting to give an animal a safe and loving home. The following information is required so that we can assist you in selecting an animal companion. When adopting an animal, it is important that your entire family knows what will be involved. All information on this application will be kept confidential unless disclosure is required by law.
Once approved and matched with your new pet, there is an Adoption Contract to sign and an adoption fee to pay. All adult animals will come spayed or neutered. If the animal is not yet, the procedure will have to be done at a later date, to be determined by The Battlefords Humane Society ( the cost of which is already covered by the adoption fee ). If you do not cooperate with the future spay/neuter, the animal may be reclaimed by the Battlefords Humane Society.

The Battlefords Humane Society will not share the information on this application for any reason not connected to the adequacy of adoption of the shelter animals or where required by law.
I understand a representitive from the Battlefords Humane Society may visit my home for an inspection before my application is approved.
I understand that if I am approved, I will need to carefully read and sign the Adoption Contract, which is a seperate document from this Adoption Application. The Adoption Contract represents the legal contract between an adopter and the Battlefords Humane Society.
By submitting this Adoption Application, I agree that I have fully understood all the questions contained herein and I further agree that all the information I have provided is true and correct.
The questions posed within this application are used to ensure that all the animals are adopted to homes that are capable of and agree to provide superior care, and who are committed to caring for the animal for their lifetime.
The Battlefords Humane Society has the right to refuse any application without providing reason.
